I'd like to ask you all for your input on a small project. I am trying to design the Geek Ticklee/Tickler Disclosure Forms.
The purpose of the forms is to provide a potential partner with a brief overview of what kind of tickler or ticklee you are, and what to expect from you during a session. You could fill one out and post it on your website or a personal ad profile. They are not intended as surveys, though you could conceivably use them as such. Being overviews, the forms should not describe you completely, but rather give the reader enough of an idea of what you are like (and what you like/don't like) to enable them to ask follow-up questions. They are supposed to jump-start the process of getting to know each other, not replace that entire process.
The purpose of this thread is to revise the forms. I want your opinions on what additional questions to include or exclude (to be useful, the forms should balance brevity and completeness), as well as what responses should be listed. The current versions of the forms follow this message. I will try to keep them current and color-code recent revisions as they are suggested. Please do not simply fill out a form and post it here -- that would belong in the Polls section, and does not contribute to the editing effort.

Common abbreviations that are used in the forms:
MOTAS (member of the appropriate sex): a person of a gender that one is attracted to (opposite sex for heterosexuals, same sex for homosexuals)
MOTIS (member of the "inappropriate" sex): a person of a gender that one is not attracted to (same sex for heterosexuals, opposite sex for homosexuals)
