He is laughing too, and his hand is flat on her belly, so it might be that one of them told a joke. It is hard to tell from a still photo whether his hand was moving.

I read somewhere that while she was filming hook she had an assistant that would clean her feet in between takes. I wish there was a video clip of that considering how ticklish she is
