I think the skin is numbed up with a local anesthetic to keep the puncture from being painful. When it wears off and the pain receptors can go back to firing at a normal rate, it'll be sore af. Though, I'm not sure how the tickling happens, I'm guessing the vibration from the liposuction travels to areas that are not numbed out so you feel the tickle there
