No, not exactly. A bishounen can simply be discribed as a handsome, cute or girly man or a man who is in touch with his feminine side. The level or point to which it becomes a "gender bender" sort of deal isn't implied in the definition of bishounen, as it would be in pictures like this, where theres a clear effort on the part of the person to look like a member of the opposite sex by crossdressing as well and having surgery, which is a whole other matter.
Some of my female friends call me bishie for fun, and I know why. Bishounen is more a state of being than an actual gender associated discriptive term. I don't want to call it quite like a trend or fashion, because thats not what it is, at least not here in America (yet), unless you count cross dressing as the equilvent. Those who do make it a trend fill up a ratio of crossdressers and bixsexuals and for that reason are not in the same loop as those who are simply handsome, cute with some girlish qualities whether in personality or in physical or acted out nuance.
There are those that go out of their way to be bishounen (because they feel its some kind of trend or fashion, like cross dressing) and those that just are bishounen (like me). I think my female friends, not to be mistaken for girlfriends since I'm not a lelch (lol) call me a bishie for that reason, I have that feminine side or qualities which makes being in their company and also being a man a non-issue. As it is, I have more female friends than male but I get along with the guys just as well.
And no one questions my sexual orrientation because everyone knows I'm a man and that I'm heterosexual, and lastly, because I don't dress up put on a show. LOL!
Its really easy to blend bishounens with crossdressers, homosexuals bisexuals, and the like, but it is not the same (even though they are almost always categorized in the same area), and this picture is clear evidence of that.
I know this explaination may not have been necessary for some and may have offended others, but I highly doubt everyone knows what "bishounen" is. And for the sake of simply explaining the difference I have. Please don't take it to heart folks. Its just food for thought.
